As I've seen on other reviews, the straps for this are absurdly over-large. I bought knowing that, and it's very easy to improvise with some cords or ties looped through the sides. It's harder to find penis shaped gags in silicone, so I purchased for that, and I find it just the right size to fill my mouth and JUST barely push back far enough to tickle my gag reflex if I'm not careful. It does make my jaw ache after a bit, but no more than any other gag would. It's a nice single piece of silicone and therefore easy to clean and no gross plastic in your mouth.
The gag itself has an appealing shape and made of a nice material. However, the strap which wraps around the head was not-so-great quality, and way too large on me even though it has velcro adjustments. It was essentially non-functional for me, which was a huge bummer. I'll have to find something else to tie into the sides.
